During the process of registering for an account, all players will be presented with the above Player Consent that must be acknolwedged that is was read and understood in order to utilize the system. The most recent version of the updated Player Consent form will be posted here for review at any time.

I, the undersigned player, voluntarily agree to participate in the use of a performance management technology platform designed to support and enhance my athletic development.

I understand that this platform will collect, process, and integrate multiple types of data related to my training, performance, and well-being, including:

• Self-reported data: Information I provide regarding how I feel (e.g., wellness, fatigue, recovery) and my perceived level of effort during training and competition.
• GPS and tracking data: Metrics such as distance covered, speed, movement patterns, and other physical performance indicators collected during training sessions and matches.
• Strength and physical testing data: Results obtained from assessments conducted using strength and conditioning equipment and related testing protocols.
• Biomechanical data: Movement analysis derived from video-based assessments to evaluate technique, efficiency, and injury risk.

I acknowledge and consent to the integration of these data sources into a unified system. I understand that combining this information provides a more comprehensive view of my performance, workload, and physical condition, which may be used by coaches, performance staff, and authorized personnel to:

• Monitor and manage my training load and recovery
• Support performance improvement
• Assist in injury prevention and rehabilitation
• Inform training and competition decisions

I understand that my data will be handled in accordance with applicable data protection laws and organizational policies, and that access will be limited to authorized individuals involved in my performance and care.

By accepting below, I confirm that:

• I have been informed about the types of data being collected and how they will be used
• I voluntarily consent to the collection, integration, and use of my data as described above
• I understand that I may withdraw my consent at any time, subject to applicable policies and agreements
